If you or your iMGA clients need to report a claim:

• Complete and submit this Claims Reporting Form, (esta forma en Español) or
• Email imga@mclarens.com , or
• Call toll free 855-411-6252, or
• Fax the claim information to 206-363-2332.

Please make sure you have all this information correct in your systems as well.
